simlpy go to the WordPress Plugin and Add new and install Ninja form which is very simple and easy.as given below image-

After install the plugin activate it. and go to to the Ninja form Dashboard and Add new form-

Add new and come back to the form dashboard.

and copy the shortcode- and create a new contact us page and select search shortcode option and then paste the short code over there.
